Adult Pricing

Ages 18+. Memberships include the ability to text your provider 7 days per week from 9 AM-7 PM and unlimited booking of telemedicine and at home visits. Price is per month and due on same day every month. Card is required to be kept on file. Enrollment fee for each adult is added to first appointment only.

45 minute, at home visit. This visit type is ideal for those who do not wish to wait in the waiting room, only to be seen for 10-15 mins by their provider. You will be treated in the comfort of your own home. This style visit can be used for sick visits, UTIs, chronic disease management, medication refills, lab ordering, and imaging ordering.

Have more than 1 person that needs a visit? Additional family members can be seen for a discounted price. Price is per person.

Telemedicine visit with your Nurse Practitioner for acute concerns, medication refills, lab orders, etc.

Sports physicals available in the comfort of your home. Please make sure to have all paperwork that needs to be signed. At this time, we do not have the ability to perform EKGs so please make sure this is not required. 

Holiday appointments can be offered based on provider availability.  All federal holidays observed. Base price + $100.

A common request is the completion of forms/paperwork for legal reasons. An additional cost will be added to all visit types, regardless of membership status. Completion of forms based on provider discretion.
