Ages newborn to 2 yr 11 mo. This cost includes all well child exams, at home visits, and telemedicine visits.  Parent (s) of child(ren) will be able to access a HIPAA compliant app to text your Nurse Practitioner 7 days/wk from 9 a.m-7 p.m. Well child visit schedule: 2 days, 1 month, 2 months, 4 months, 6 months, 9 month, 12 months, 15 months, 18 months, 24 months, 30 months.

*AYDSM does not carry vaccines so all vaccines are free and will be administered at Maricopa County Clinic

Ages 3-10 years. This cost includes all well child exams (yearly), at home visits, and telemedicine visits.  Parent (s) of child(ren) will be able to access a HIPAA compliant app to text your Nurse Practitioner 7 days/wk from 9 a.m-7 p.m. Well child visits are performed yearly starting at 36 months (3 years old).

There is a cap placed for families that have more than 3 children in effort to provide affordable care. 

45 minute well child exam with a Nurse Practitioner. Exam includes age appropriate screening. Well child visit schedule: 2 days, 1 month, 2 months, 4 months, 6 months, 9 month, 12 months, 15 months, 18 months, 24 months, 30 months, and yearly starting at 36 months.

Same day 45 minute well child exam with a Nurse Practitioner. Exam includes age appropriate screening.  Well child visit schedule: 2 days, 1 month, 2 months, 4 months, 6 months, 9 month, 12 months, 15 months, 18 months, 24 months, 30 months, and yearly starting at 36 months.

This visit type is designed for pediatric complaints such as ear pain, upper respiratory infections, fevers, nausea/vomiting, diarrhea, etc. 

15 minute pediatric telemedicine visit. This visit type is most appropriate for pediatric complaints that can be diagnosed without specific diagnostic tools. Availability will be determined based on provider's comfort level.

Have more than 1 person that needs a visit? Additional children can be seen for a discounted price. Price is per person.

Sports physicals available in the comfort of your home. Please make sure to have all paperwork that needs to be signed. At this time, we do not have the ability to perform EKGs so please make sure this is not required. 

Holiday appointments can be offered based on provider availability.  All federal holidays observed. Base price + $100. 

*A fuel charge of $10 will be added to at home visits > 20 miles in distance 

A common request is the completion of forms/paperwork for legal reasons. An additional cost will be added to all visit types, regardless of membership status. Completion of forms based on provider discretion.
