We are a mobile, nurse practitioner-run concierge practice serving patients across Scottsdale & Phoenix, AZ. We bring personalized healthcare directly to your home or connect with you via telemedicine — eliminating waiting rooms and rushed appointments. Our practice is led by Cheyenna Kopacz, DNP, FNP-C and a team of Nurse Practitioners committed to a root-cause approach to your health.
We are a cash-pay practice and do not bill insurance directly. However, we can provide you with a superbill after your visit so you can submit it to your insurance for potential reimbursement. We also gladly accept HSA and FSA payments. Please note that visit notes cannot be modified once finalized and a superbill is issued.
As insurance deductibles continue to rise, concierge care is often more cost-effective for patients. Without the pressure of high patient volumes, your provider can spend genuine time with you — not just manage symptoms. In-home and telemedicine visits also eliminate long office wait times, putting your schedule first.
At this time, we do not offer payment plans. Payment is due at the time of service. Our membership options are designed to make routine care more affordable and predictable month to month.
We provide in-home visits across Metro Phoenix including Scottsdale, North Phoenix, Peoria, Glendale, Tempe, Chandler, Gilbert, Mesa, and surrounding areas. A travel/fuel fee of $10–$50 may apply based on distance from your provider. Telemedicine is available statewide in Arizona.
Our Scottsdale office is located at 7825 E Gelding Dr, Suite 104, Scottsdale, AZ 85260. We serve patients throughout Metro Phoenix and Scottsdale — and with our mobile care model, we can also come directly to you wherever you are in the area.
Yes. We serve patients throughout Utah via telemedicine and select in-home visit areas. Our Utah patients receive the same personalized, root-cause focused care as our Phoenix-area patients — without insurance requirements and with direct provider access.
No. If you are looking for an insurance concierge (a brokerage service or advisor to help you find, compare, or purchase commercial health insurance plans), that is a different industry and a service we do not offer. At Your Doorstep Medical & Concierge Care is a mobile concierge medical practice. Our team of board-certified Nurse Practitioners delivers actual medical care—including family primary care, medical weight loss, and hormone therapy—directly to your home or office.

Adult membership is ~$99/month and includes unlimited telemedicine visits, unlimited at-home visit bookings, and 7-day-a-week provider texting access (extended to 7:00 PM for members). A one-time enrollment fee of ~$50 applies for new clients.
Pediatric membership fees vary by age — for example, children ages 0–2 years are ~$160/month. Families are capped at ~$355/month regardless of how many children are enrolled, making it cost-effective for larger families.
Yes. Our dedicated hormone program membership is ~$150/month and covers ongoing management of Testosterone Replacement Therapy (TRT) for men and Hormone Replacement Therapy (HRT) for women, including follow-up consultations and care coordination.
Absolutely. We offer à la carte visits with no membership required: at-home visits are ~$199 and telemedicine visits are ~$115. A one-time enrollment fee of ~$50 applies for all new clients.
Yes — we treat newborns through teenagers. Pediatric services include sick visits, well-child exams, chronic condition management, and sports physicals. We make it easy for the whole family to access quality care from the comfort of home.
Yes. Functional medicine focuses on identifying and addressing the root causes of health issues rather than simply managing symptoms. We use comprehensive lab testing to investigate concerns like persistent fatigue, brain fog, gut health imbalances, and hormonal disruption — then build a targeted care plan around your results.
Yes. Our medical weight loss program includes personalized plans and access to FDA-approved medications including GLP-1s, when appropriate. All plans are developed with your health history and goals in mind — not a one-size-fits-all approach.
We do not require vaccines to become a patient. We do encourage open conversations with your Nurse Practitioner about immunizations — our goal is informed, collaborative care. Please bring any questions to your appointment.
In addition to primary care and functional medicine, we offer: IV hydration therapy, trigger point injections, sports physicals, chronic disease management (diabetes, hypertension, etc.), and sick visits for common conditions like UTIs and infections — all delivered at home or via telemedicine.
Our standard hours are Monday – Friday, 8:00 AM – 4:00 PM. Members enjoy extended provider texting access until 7:00 PM, seven days a week — so you're never without guidance when something comes up after hours.
Yes! We have multiple Nurse Practitioners on our team, which allows us to accommodate a wider range of scheduling needs. Contact us directly to discuss availability outside of standard hours.
It depends on your care needs. If you are managing an ongoing condition or taking medication that requires refills, routine follow-up visits are recommended. Your provider will discuss a care schedule that fits your situation. Our membership plans make frequent visits significantly more affordable.
Yes. A fuel/travel fee ranging from $10 to $50 is added to in-home visits based on the distance from your provider's location. This will be communicated transparently at the time of booking — no surprises.
